Assertive Community Treatment (ACT) is an evidence-based treatment model providing multidisciplinary and comprehensive care to individuals experiencing severe and persistent mental illness who also experience co-occurring difficulties such as substance use, homelessness, legal involvement, medical problems, and other such high-risk complications. ACT works with a rehabilitative focus to restore the highest level of community functioning possible across multiple domains. ACT is delivered in the community, in people’s own homes and personal environments, where learning can be directly applied, and all services are assertive, so that individuals having difficulty engaging and participating with care can still receive support. Specialized services which are available from within the team include psychiatric medication management, nursing care, peer-based supports, substance use services, supported employment, and supported housing/tenancy support. The ACT team carries its own crisis phone, and services are available 24/7, 365 days a year.
Thank you for existing. You helped me make a hard transformation during a very stressful period of my life. I don’t know where I would be without this service program. It made me feel welcome, safe, and secure.
Dr. John Carter has been seeing my boys since small children. I love how he includes the parent in last half of sessions, and explains everything as clearly as he can, and stays on top of medication, and side effects.
I contacted family preservation services at an extremely difficult time for my family and I. They have been amazing, from the variety of therapists and flexible hours. Just the overall care and concern has been just what we have needed. The search for therapists is very difficult but I'm so blessed we have found them.
My brother has been a patient here for years now and they have been there for my family in more ways than not. They are great here at this clinic and my family and I are very grateful to the entire staff!
